I will need to start billing OP psychotherapy (90832, 90834, 90836) and psychiatric assessment codes (90791) for a facility/group. The professional services are rendered by employees of the facility, we want the reimbursement to go to the facility and the facility then pays the individual provider. Do I bill this on a UB-04, or do I bill on a CMS-1500 and use the TIN for the facility?
