I am in dire need of my fellow coders and instructors help in finding examples of physician notes for family practice. I would like them to of several types that have good chief complaints and in general great documentation. If anyone could supply these within the HIPAA guidelines I would greatly appreciate your help. I am in need to help local physicians and staff showing them correct documentation examples and WHY it is needed.
