Family conference.

The patient is not present for this visit. Her son is here and is talking about the use of a PEG tube. I am told that the hospice worker tried to contact us last week about a PEG tube since this would obviate her aspiration. Her son has talked to her about this in the past and we will see if we can expedite the process. The patient is currently in hospice and not here.

IMPRESSION:
1. Hypertension.
2. GERD.
3. Hypovitaminosis D.
4. Dementia.

We spent 15 minutes with the patient's son.

Medicare patient?

If the patient is covered by Medicare you cannot code for any service that is NOT provided face-to-face with the patient present. Yes, even when the patient is unable to participate in the conference, you have to speak to the family (or decision maker) in the presence of the patient.
