If a therapist performs individual therapy for 30 minutes (90832) and immediately afterwards performs family therapy (90847) for 30 minutes, can we bill both codes? Do we need to add modifier 59?

I ask because it is our understanding that we cannot bill and be paid for more than one therapy in a day.

I appreciate feedback.

Please see this LCD article from medicare to see if this gives you any clarity.

In the practice that I work for we would only be billing the 90847 most likely. The question really comes down to who is the patient and what is the focus of the treatment. Our sessions are 45 minutes and we schedule the individual therapy on separate days from the family therapy.

Per the CPT book, psychotherapy can be performed with patient and/or family- i would just bill the 90837
