Hello,
I am needing help UHC Medicare Advantage is rejecting a claim with code J9395 It is administered on the right and left buttock. This is the rejection I am receiving from UHC the JZ or JW modifier submitted is inappropriate.
This claim has been rejected and will not be processed. This is what my claim look like.
