We're using Lupron (leuprolide acetate-J9217) for fertility preservation in one of our patients having chemotherapy. The insurer is denying, stating that the diagnosis is not in the list of indicated diagnoses. Has anyone else had the same or a similar issue? We were approved to administer the drug and now we're hitting a brick wall. Any help would be great. Thanks!
