patient was 20.5 weeks pregnant with contractions and bleeding. She is 42 years old and had a tubal ligation reversed. Doctor documented fetal demise with manual delivery of "fetus in the amniotic sac with placenta intact." There was no inducement, no D & C and ultrasound confirmed no retained products of conception. Please help with all diagnoses and cpt code.
