Wiki Fixation failure - Repeat procedure?

Hi,

My provider treated a hip fracture 2 months ago with an IM nail. The patient is now being brought back to the OR, due to fixation failure, and having the IM nail removed and a new one put in. How would you code this?

the dxcode would be a mechanical complication of ortho implant and you bill the procedure with the 78 modifier
