I have a physicians that order a flex but he enter the scope into the transverse colon per the decision tree this should be a incomplete colon but i am question if it should be a complete flex since he did order a flex and intended to do a flex?
 
You would code the procedure that was actually performed. Physician often have to do a more extensive procedure than what they originally intended if the finding indicate it. That doesn't mean that complete procedure should not be coded. Ancillary staff cannot change what was ordered, but a physician is allowed to make changes to the plan if they decide it is necessary.
