If a patient have a Private Primary insurance and a medicaid secondary insurance are you suppose to bypass the primary insurance and only bill the secondary insurance for the flu shot?
 
Medicaid is always the payer of last resort, so you'll want to bill the primary insurance to get the denial to bill Medicaid. Otherwise, you would get a denial from medicaid more than likely stating "other payer primary".
 
Does it matter if it is in a hospital setting? If it's a state stocked vaccine can you still bill a private insurance or do you bypass the primary and only bill medicaid (straight, OFC or HKP?)
 
you still have to bill the primary first for any service when on MA, they will deny for Coordination of Benefits otherwise
