Wiki Flu shot rejections, please help

I need help with billing flu shots. I've been out of Family Practice for 10 years.

I am getting rejects for unit of measure on 90662. Does the unit of measure need to be 1 in or ML0.5 ??
Do most insurance companies require the NDC number also.

Most of our claims require the NDC number now. Also, are the flu vaccines one year apart? I have had a few deny because of this. I am sure you know the 90662 only applies to 65 and over.
