I know if my dictation for a pacemaker or ICD mentions fluoroscopy I can use 71090-26,

what about heart cath, dictation says "with some difficulty getting the Swan-Ganz to the right pulmonary artery fluoroscopically"

already included in 93503 or seperate 7 code or don't code ?

76000-26 (up to 1 hour physician time). According to CMS CCI edits it is not included in 93503. It seems to be included in every other cath placement code though.
