Patient was in hospital with 2 unsuccessful attempted colonoscopies due to severe bleeding. Third attempt successful with treatment of argon plasma coagulation. Dr. ordered patient return in 8 weeks for follow up colonoscopy. We used V67.59 and 45378 but it was denied for medical necessity. If the doctor doesn't know that it's healed, which is why he's doing the procedure, then how can I use follow-up and no "sick" diagnosis?
