Can you bill a foot exam and retinal eye exam on the same day with an E/ M code or without E/M code?

For our clinic, I believe the foot exam would be part of the E/M exam, so I'm not really sure what advice to give on that. But we bill the retinal scans on the same day. And I don't believe we put a modifier on the E/M for it. But I do know that UHC Medicare replacement plans haven't been paying for it at all, but the commercial UHC does. Other insurances have been paying.
