For an adverse incident report I need the correct Surgical description ICD10 codes for Implantation of wrong lens, Treatment Description ICD10 for removal and implantation of correct lens ( CPT 66986) in Ophthalmology as well as the resulting injury (None)

if intra op H59.88. misadventure code either Y62.51 or Y65.8. Code Y92.234 and Y77.2, Z96.1
