Good Afternoon!
We bill Part B of A for FQHC LCSW claims to Medicare. We are getting denials, but very vague in explanation of course. We bill 96150 with an AJ modifier and a 59 if the service was on the same day as an E/M with one of our providers. We are billing with a 771 bill type on UB form.
We are on Connex (online working of claims for Medicare) and they just keep saying "sent back to provider" with no other explanation. The customer Service line is really no help to us.
My question is, does anyone know of any other way to get these claims paid? Anyone had similar experience with LCSW claims?
