Hello, I am looking for any guidance on diagnosis coding for Ob prenatal visits. We are an FQHC clinic that does OB care. We do not bill for the delivery. Curious to know how other FQHC facilities record their diagnosis for ob visits? For example, if pt has Obesity and previous cesarean do you record those diagnosis on every visit?

Interested in your question. We are FQHC as well. I am curious about not billing for delivery and the diagnosis'. We are currently getting denials for Z34.82.

We are an FQHC provider and we code and bill for OB Services and not the delivery. We do code for obesity for each visit if the weight of the patient is documented and the physician provides a notation or assessment in regards to the obesity. In regards to the previous C-section scar, we only code this when it is addressed by the provider as point of care or assessment in regards to the up coming delivery.

We have not had any problems with Z34.82? Our facility is located in California.
