1.when a patient come in for fracture treatment, do you bill e/m-25,fracture code,supplies,x-ray?

2. how do you bill when they come back for a replacement cast,strappling,splint?

Hospital or Physician coding?

Please specify if you are coding for the hospital or for the physician.

If billing for the physician - in general for fracture care initial visit you'd use -57 modifier (decision for surgery) as fracture care codes - even closed treatment - are considered "major surgery" (90-day global). Follow-up visits would be 99024 NC F/U.
