If a provider has already seen a patient for a fracture and charged an E&M for the initial visit can the provider bill for fracture care for the follow up visit?
 
In my opinion no. They have to pick either the "itemized" E&M route from the onset or the fracture care. I have had this discussion before where they state they were waiting to see if it displaced or not. It's just questionable to charge the global after they have already charged an E&M the first visit for it. Just stick with the E&M non-global route. Aside from the coding implications you will probably have a very unhappy patient if they had a copay for the first visit and then you charge them a global and it hits their deductible and they see the larger fee due to it being in the "surgery" section of the CPT book.
