Wiki Fracture Care, Splints and Casting?

When our physician's see a patient with a possible arm fracture, they will usually splint the arm & send the patient off for x-rays. If they do have a fracture, the patient comes back and the physician applies the cast.

My Question is: since we apply a splint and then the cast, can we bill for one of these? The CPT book states that fracture care includes "the application and removal of the first cast or traction device only".

In the past we would bill the 1st visit as fracture care, E/M with modifier 25and the supplies for the splint; the 2nd visit we just bill the supplies for the cast. Now the Doctor wants to change that & bill for both the cast & supplies on the 2nd visit.

Any help is appreciated.
