I code for a radiologist. A knee x-ray was done for "pain" Findings state "Fracture of the proximal fibula. Age uncertain". Would the 7th character be S for Sequela?
 
It is an A only if the patient never sought treatment for the fracture and is now presenting to receive treatment for the fracture. If the pain the patient has is not by documentation being related to the fracture then you would use the M code for pain in knee. Just because the radiologist documents evidence of previous fracture does not automatically make this an encounter for the fracture or the pain due to the fracture.
