So it seems some examples I have read while studying on Fraud and abuse are very similar.
Is the core difference Knowing vs.not knowing; intentional vs not intentional?

Fraud; Billing a higher level of service than medically necessary

Abuse; Improper billing practices

These were both examples provided.
