Has anyone seen any new information concerning the Functional Therapy Codes as to how often they must be attached to the therapy visits beginning Oct. 1. Medicare has told us that they must be applied to every visit, however all the information we see says to bill them on the initial visit, every tenth visit and on discharge. Thank you.
 
So we were also told that we need report the Gcodes on each claim. One important thing that we learned after receiving frequent denials is that the GP modifier needs to be added to the Gcode as the primary modifier before the severity modifier. Hope this helps!:)
