Wiki fup visit and critical care same day- how to bill

Dr. saw the patient in hospital as fup and then same day saw pt in critical care.

Can I post one for fup and other for critical care depending on time.

Is there a modifier for it.We are internist.

Input helps.

According to the CPT book under Critical Care, it states
"Critical Care and other E/M services may be provided to the same patient on the same date by the same physician."
The documention for the hospital followup is determined by 2 of 3 key components.
The critical care is based on time, which needs to be documented in the chart.
There is no modifier so you should expect to have to appeal if it is denied.
