Where I work, we do count it as part of one of the 7 elements if the doctor documents it as being surgically absent

Also you need to add the V88.xx code for the absence of uterus and cervix, and if a PAP was performed and the patient does not have a remaining cervical stump then you need to add the V76.47 for vaginal PAP.
