Does code G0289 require the loose body to be over 5mm? I've heard that but cant find a factual reference to support it.

AAOS Global Service Data states "arthroscopic removal of loose or foreign bodies is separately reportable if greater than 5 mm or removed through a separate incision or portal," however, NCCI Guidelines do not have the 5 mm piece. Per NCCI Guidelines, G0289 may only be reported for removal of a loose body or foreign body from a different compartment of the same knee.
