Can someone please give me an example of how to bill a doctor's visit with a urine drug screen that is CLIA waived? We are a pain management clinic and the billing people to date have not been billing the urine test we do. The doctor's visit is usually a 99214 for established patients or a 99205 for new patients. Now, for Medicare would it be the 99214 with a modifier of 25 and then the G0434 with a modifier of QW? Help? :confused:
 
yes you can bill the two codes together-
99214 no modifier needed (if documentation supports it)
G0434QW

Make sure that G0434 is the appropriate code for the testing you are performing. G0434 is for multiple drug testing and will be used to report very simple testing methods, such as dipsticks, cups, cassettes, and cards, that are interpreted visually, with the assistance of a scanner, or are read utilizing a moderately complex reader device outside the instrumented laboratory setting.
