Wiki G0447 and mod 59

I code for a weight loss clinic, we have been coding The E&M visit (99212-99214) with G0447 mod 59 since I started 3 years ago.
Now we are getting them all denied from BCBS and Tufts- several long hours spent on the phone but no help from the ins carrier at all.
I have tried a few different modifiers, with no luck.
Anyone have any insight for me?

I don't think a -59 modifier is correct here.
I would code this as:
9921* -25, G0447.
