Medicare just changed the LCD for these two codes effective Aug 1. The only diagnosis that is now considered medically necessary for us to use is V16.0 which is not always appropriate for the esophageal biopsy we are doing in a patient with gastritis when we suspect H Pylori. What should we be using:confused:?
