Wiki G0463 and E/M visits

I have an hospital out patient clinic that is billing a non-Medicare patient using both the G0463 and an E/M code (99204, 99205, 99214, 99215). What are the general rules? 1: Can a hospital outpatient clinic bill the G0463? 2: can they bill both the G0463 and the E/M for the same provider on the same date of service? 3: Does anyone have or can point to documentation regarding these matters?
