the NCD states 22 visits per year and breaks it out to 1 visit per week for the first month, 1 visit every other week for months 2-6, at 6 month eval, if patient has lost 6.6 lbs you can continue with 1 visit per month for months 7-12. My question is, if in month 1 the patient only makes 2 visits, does that disqualify the patient, or can we continue the program billing the 1 visit every other week for months 2-6? I have a provider worried that since the patient missed two visits Medicare will not pay for any of the remaining visits of the benefit.
