One of my providers is asking questions about using the G2211 modifiers for her biologic patients. She is saying that all the "behind the scenes" work her nurses do like prior auths for the meds and calling out the prescriptions should count towards our determining factor to use the code. I however struggle to get her to understand that all the work needs to be included in her documentation. Here is an example of one of her visits.

Assessment & Plan​

1. Acne severe nodulocystic- improved on 1 month accutane. Start accutane 40mg BID. School note about nausea is a side effect of medicine. He has been taking for 1 month. Patient can take zofran at school. Start zofran 4mg every 6 hours as needed for nausea.
Follow up in 1 month or sooner if needed

  • Check CBC (includes diff/plt)
  • Check lipid panel with reflex to direct LDL
  • Check hepatic function panel
  • Change Accutane 40 mg capsule from TAKE 1 CAPSULE BY MOUTH DAILY to TAKE 2 CAPSULES BY MOUTH DAILY
nausea
  • Start Zofran 4 mg tablet TAKE ONE TABLET BY MOUTH EVERY 6 HOURS TO AS NEEDED FOR NAUSEA
