I am requesting clarification regarding billing of CPT G2211 in conjunction with an office/outpatient E/M visit and a procedure performed on the same date of service by the same provider.
Is it acceptable to:
Report an E/M code (99212–99215 or 99202–99205) for chronic condition management,
Append CPT G2211 to reflect visit complexity/longitudinal care, and
Use modifier -25 on the E/M code to indicate that a separately identifiable procedure was also performed during the same encounter?
