Patient presented w/anemia, gastric ulcer. Provider stated Gastric Ulcer; GI Bleed, however, after enteroscopy, provider stated "two superficial non-bleeding gastric ulcers, however, w/stigmata of bleeding". Would that statement allow for a gastric ulcer w/hemorrhage to be coded? To me, since the test results state "non-bleeding", I would think the code would just be gastric ulcer. thanks for any feedback.
