Our physician documents on a patient's wellness visit (non-medicare). "Patient is here for a general wellness. Please see the face sheet, I've updated his medications, social, family history, previous surgeries, and allergies." Is this sufficient documentation? Seems to me he should state any changes and if no changes should state no changes. What are your thoughts? Thanks
