We are begining to do geriatric assessments in our facility. We have a clinic attached to a hospital & rehab center. Our physician in the clinic will perform the physicial examination. The OT & PT will also do their assessment. Do we code the physical examination with an established problem visit code as the reason for doing the assessment is due to the patient's dementia and ataxia?
Any help would be appreciated. Thanks
