Wiki Germline Genetic Testing Insurance Coverage?

Trying to establish a list of insurances that have a policy related to germline testing for hereditary cancer (work in Urology- prostate cancer). Anyone have anything on this? So far I've only been able to find applicable policies for Medicare, UHC, Humana and Cigna on their websites. Any help is appreciated!
