Hello,

Patient had 33284 and 33208 KX for I49.5. Then the cardiac electrode was displaced so patient had 33235 78 and 33216 78 59. 33216 is being denied inclusive to 33235. The insurance is BCN. Any suggestion on how to 33216 paid ? Is there a different modifier that could be used ?
