Question GI Denials Regarding ERCP and EUS

Good Morning Everyone,

I work for a GI specialist and we have been billing ERCP and EUS codes together. We have insurance companies coming back and denying cpt code 43237 (only this specific codes). The listed information down below is how we coded the procedure.
Does anyone have any insights on why they have denied the cpt code 43237 and or if we can even bill this code with the other codes? I can't find any evidence to support not billing all of them together.

CPT: 43264-59
CPT: 43237
CPT: 43273-59
CPT: 74328-26
