can you code an extra level for a gill if there is more than 1 level? ex: L3-L4, L4-L5. would it be 63012 & 63047 or do you just bill the 63012 once no matter how many levels it is?

Its one code includes multiple levels. When you look at the code description you will notice the words facets, pars inter-articularis, nerve roots. The s at the end means one or multiple.
