My doctor did a Gill procedure at L5-S1 (63012) and he also did a Gill procedure at L4-L5. How do I code for the second level? Would I use 63012-59?

I believe you can only code it once. Read the desrciption in "The coders Desk reference" , it states:
the physicion resects the spinous process of all three vertebra and the middle part of the loose fith lumbar neural arch..
 
You can only code this procedure one time. In the description of the procedure in the Ingenix coding companion for Neurosurgery it states that the Gill procedure involves removal of spinous processes of L4,L5 and S1. The work done at L4-L5 is included in the code.
