If Primary insurance applied global delivery to deductible and Medicaid will only pay for delivery only (not global) how would I collect for the antepartum visits if MCD does not pay for 59400?
 
I don't know your state Medicaid ruls, but in WA, if you bill the global to the primary insurance, then submit the same codes, CPT & ICD-9, to Medicaid, along with the EOB from the primary and they will pay the rate per the state fee schedule.:)
