Debate on global billing

How do we bill if we see a patient for her entire pregnancy but she has an insurance change during the pregnancy? If we see her for 7 visits under the first insurance we would bill 59426. The confusion is how to bill for the new insurance, do we bill globaly or do we bill for the visits and delivery separate?
