if a patient has a PPM placed on Tuesday, and returns (planned) Wednesday for an AVJ, does the AVJ get a '58' modifier? OR a '79'?

Yes, most likely modifier 58 is correct but review your documentation to make sure it's supported. Keep in mind that 58 means not just 'planned', but 'planned prospectively or at the time of the original procedure'. But if the documentation indicates that second procedure is due to a complication, then modifier 78 is more appropriate, or if due to condition unrelated to the first procedure, then modifier 79. But it's just a technicality in a case like this because it's going to fall outside the global package one way or the other.
