Patient with Acute Renal Failure (+another chronic conditions) was in Hospital on inpatient status for almost a month.
We are practice of 30 providers - Nephrology.
One of our providers - surgeon was called in hospital to perform 36558 (cath. insertion), and than 12 days later to preform 36581 (cath. replacement), to get dialysis treatment for patient.
Patient was seen almost every day by our another providers (for mostly 99232) to monitor his Acute Renal Failure.
Now we got denial for dates of service after each procedure - stating: this procedures have 10 days global period, and those another providers are part of same practice.
Is there any way how to bill those claims to get paid from Anthem insurance? Mod. -25?
Please help me with this.
