Not sure if it's taboo to ask, but does anyone have an inpatient OB billing form that they are willing to share. I'm trying not to reinvent the wheel, but I don't want to start from scratch either.

I'm trying to help my OB provider get away from the "Adobe Acrobat" nightmare version that is NOT working for me! :/
