Wiki Grammatical errors in the provider note

I am looking for some input on grammatical errors in the provider notes. Some errors are misspellings, others are that there is a gender issue where there is a "he" in the note instead of a "she" and vise versa. Do these grammatical errors have to be corrected in order for the note to be valid? Any input would be greatly appreciated.
 
Errors do not make a note invalid.

It would be better if they were corrected. I know that when I see that sort of error, it makes me think the provider is not paying attention to me. If he gets that wrong, what else is he getting wrong about me?
 
The grammatical errors do not invalidate the encounter note. I would suggest a review of the templates, sometimes the error is there. Sometimes it is a scribe that is entering the data and just needs to be educated on proper spelling and attention to detail. I agree with Sharon, it is a poor reflection on the provider. Find out if the issue bothers him or her, for some providers it is not an issue.
