Physician performed bilateral nephrostograms with attempted but unsuccessful bilateral ureteral stent placements and then replacement of bilateral percutaneous nephrostomy tubes. Should I code the attempted stent placements with a modifier or only code the nephrostograms and tube replacements? Thanks!!

If the nephrostograms are full and complete, not just the tubes injected, then bill for the nephrostogram. Otherwise code for the exchange only.
