The FDA approved the use of Guardasil up to age 45 in October 2018. Is anyone receiving payments for patients who receive it age 28-45?

We don't give Gardasil in our practice, but I do know this is an issue with most carriers. While the FDA guidelines changed, most carriers have not changed their policy yet. I would advise creating a standard letter to submit appeal along with the updated FDA guideline. You may also want to consider having your patients sign ABNs, or no surprise billing forms to make them aware prior.
