The Dr. here done an outpatient procedure of a TLH BSO I will use 58571. But she also done a Lysis of adhesions (LOA), is there a separate code for this procedure and which modifier?

I would look at that 58660 and a modifier 59.

CCI edit

58660 is a CCI edit with 58571 that they may never be billed together. If the LOA was extensive (and documented in the op note as such), you can try 58571-22. It won't fly if the note says something like - "took down filmy adhesions". It may if the op note states something like "extensive adhesions of the left ovary and fallopian tube to the omentum taken down with careful sharp dissection over 45 minutes".
